Please remember to inform the Practice if you move address or change your telephone number.  Even if you are not currently receiving treatment from the Practice an occasion may arise when we need to contact you.

Please remember to inform us if you change your mobile telephone number as appointment reminders are sent via SMS message.

It is also important to remember to inform the Hospital if you are waiting for an outpatient appointment or are currently under their care.
