You can use this service to seek medical advice from a clinician online.  Please only use this service if you feel your condition can be suitably assessed and discussed online.  DO NOT USE THIS SERVICE TO REQUEST A FACE TO FACE OR TELEPHONE APPOINTMENT, if you wish to consult a clinician by telephone or in a face to face appointment, please call the Surgery on 01323 727531.

Please ensure you complete all questions presented to you prior to submitting the consultation, to avoid having to resubmit information to us. We require this level of detail to be able to help you safely.

You can contact our admin team online to request sick notes, prescriptions, test results and other administrative queries.
